REHABILITATION MEDICINE
Overview
Rehabilitation medicine forms an important phase of medical care in hospitals and other health care facilities. Rehabilitation helps in providing an improved quality of life through its preventive and curative concepts. Today every medical facility has doctors, surgeons and nurses, as also physiotherapists, occupational therapists, audiology and speech therapists [Refer: Audiologist], mental rehabilitation specialists who help the patients get back to a fruitful life after disease, injury or any other setback to good health.
Accepted as an integral aspect of medical care the field of rehabilitation medicine throws up options that are able to offer good emoluments, social standing, prestige, commitment to life and the living. Involving a shorter period of training and improved prospects of employment, both in India and abroad, physiotherapy, occupational therapy, etc., are attracting many students. Doctors and surgeons too are depending more and more on a full-fledged rehabilitation programme involving the services of these experts. Rehabilitation therapists play an important role in restoring persons with continuing impairments to optimum levels of performance in their homes and communities, thus enhancing the overall efficiency of our health care systems.
Rehabilitation Council Of India (RCI) is a statutory body of the Government of India. It regulates and monitors the training of professionals and personnel in the field of rehabilitation for persons with disabilities. All institutions conducting training courses in the field of rehabilitation are required to be recognised by RCI.
Nature of Work
A therapist is a part of the medical team which includes doctors, nurses, social workers, diagnostic technicians, psychologists, etc., all of whom are committed to saving lives and making living worthwhile. Therapists use a variety of techniques and devices to help patients who are injured, physically disabled or mentally or emotionally disturbed or afflicted with speech disorders or hearing problems. Services of rehabilitation medical experts enable patients to lead meaningful lives.
Physiotherapists use exercises and movement, electrotherapy, manipulation and massage to treat the injured, disabled, sick and convalescent of all ages for a large variety of conditions. They are responsible for analysing and assessing patients' conditions and for planning their treatment. They perform and interpret tests and measurements for muscle strength, motor development, functioning capacity, and respiratory and circulatory efficiency Most patients are referred by doctors who give physiotherapists the information they need, but physiotherapists in private practice also treat self-referred patients.
The work of a physiotherapist can be physically strenuous as it involves lifting and supporting patients, but physiotherapists learn to lift heavy weights without strain. With an aging population, the treatment of physical injuries and old-age maladies through massage and regulated exercise and water, light, heat and electricity will require more physiotherapists. The field is getting more technical.
Respiratory therapists evaluate, treat and care for patients with breathing disorders. In evaluating patients, respiratory therapists test lung capacity and analyse oxygen and carbon dioxide concentrations and hydrogen potential, a measure of acidity or alkalinity of the blood. They treat all kinds of patients—premature babies whose lungs are not fully developed as well as elderly people whose lungs are diseased. They lay the result of their tests before the doctors on the case. Their training may be a part of physiotherapy training as a specialisation. It is definitely going to be an area of great need in the next century as we see a growth in the elderly population, more AIDS patients and increase in pollution-related respiratory problems.
Occupational therapists are concerned with assessing and treating people with a wide range of physical and psychological problems. The emphasis is on the patient's ability not disability. Their goal is to help patients have independent, productive and satisfying lives at work, leisure and in daily living, despite illness, disability or old age.
Mental retardation training teaches students how to help patients lead useful and productive lives.
Work Environment
Most physiotherapists and respiratory therapists work in hospitals and rehabilitation centres. Physiotherapists find opportunities for private practice, or work in sports and physical fitness centres and special schools. Their work hours are quite regular but reputed physiotherapists usually work longer hours offering their services to a number of health facilities, working part-time in each.
Occupational therapists work in hospitals, schools, in voluntary organisations, in people's homes, in private practice and in rehabilitation centres. They normally work regular hours and may divide their workday among a few health facilities working part-time in each.
Specialisations
Sports physiotherapy is an emerging specialisation expected to witness further growth. Respiratory therapy, which is taught usually as a part of the physiotherapy course, may eventually gain more prominence. Many physiotherapists are setting up as fitness consultants while others are establishing pain clinics.
Prosthetic and orthotic engineering teaches how to help patients with physical defects in their anatomy or those who have lost full use of their limbs due to disease or injury It trains you to make aids and devices which can help such patients with their movements. It is an allied profession, which requires technical skills with attitudes of a medico.
Physiatry or physical medicine & rehabilitation is allied to higher education in medicine. Physiatrists are physicians who specialise in the diagnosis and treatment of patients with muscular, neurologic, cardiovascular and other body impairments and disorders. In this branch of postgraduate medical education (leading to the MD/equivalent), the object is to restore the patient's physical, mental and social well being through various methods or modalities of treatment. The work is primarily hospital-based and involves helping to restore stroke and accident patients to health or helping patients suffering from a variety of neurologic disorders such as multiple sclerosis, etc., to maximise their lives and ability to function. These specialists also treat disorders involving arthritis, lower back pain, and head and spinal cord trauma.
Physical and Psychological Requirements
Aspiring rehabilitation medical professionals should have the capacity and willingness to work hard, possess the zeal to serve and an abundance of patience. A cool and calm temperament, and boundless energy and empathy with the patient are important qualities to have or to cultivate if you plan to join these fields of medicine. You must be observant, resourceful, tolerant, be able to communicate with people, combine firmness with kindness and above all, have a genuine interest in people.
In order to deal with patients with mental retardation, you must have unlimited patience, perseverance, sympathy and an encouraging and positive attitude.
The job of physical therapists or physiotherapists can be physically demanding because they often have to kneel, crouch, stoop and stand for long periods of time. They may have to move heavy equipment and lift patients or help them turn, stand or walk.
The work of an occupational therapist can be tiring because they are on their feet most of the time.
Study/Training
Physiotherapy/occupational therapy
The four-and-a-half year BSc. (Physiotherapy/Occupational Therapy) course is open to students after Plus Two or equivalent exam. Your subjects should have been Physics, Chemistry and Biology and at least 50 per cent marks in the aggregate. English also should have been one of the Plus Two subjects. The Master's degree course in Physio therapy/Occupational Therapy is for a duration of 2 years and only those who hold the degree of BPT/BOT respectively can apply for admission.
Physiatry/physical medicine
The MD in Physical Medicine & Rehabilitation is offered only after the MBBS degree. It is meant for those who wish to become physicians specialising in the diagnosis and treatment of patients with muscular, neurological, cardiovascular and other body impairments and disorders.
Prosthetics & orthotics
Bachelor/Diploma of Prosthetics & Orthotics is only available to students who have completed 10+2 with Physics, Chemistry, Maths.
Mental Retardation
The one-year diploma course in Mental Retardation is meant for those who have completed 10+2 or equivalent exam/graduation and are abovelB years old.
The 3-year Bachelor's degree course in Mental Retardation offers admissions through an Entrance Exam.
Applicants should have completed Plus Two in any stream with 50 per cent marks and be at least 17 years old.
Graduates are also encouraged to join.
Admissions to all the above courses is usually based on certain standardised eligibility criteria and on the performance in entrance examinations.
Employment Prospects
The acceptance of rehabilitation medicine as an integral part of comprehensive medical care has created a very vibrant and meaningful niche in the medical field. Physiotherapists, occupational therapists, respiratory therapists, mental retardation specialists and physiatrists usually find placement in hospitals, nursing homes, clinics, rehabilitation centres, sports coaching centres (sports physiotherapy) and in homes of patients. The nature of work would range over areas of diagnosis, treatment and therapeutic follow-up.
The type of assignments you take up after the course would depend on the experience and exposure you have received during your training and internship as well as on the employment environment you are interested in and its requirements. While regular employment fetches starting salaries in the range of Rs. 5,0007- the figure varies with the employer and years of experience. The major earnings are in consulting practice.
These options open up innumerable employment opportunities both in India and abroad as these courses are also given recognition overseas. There is a huge demand for our therapists in other countries too.
Private hospitals, nursing homes and diagnostic centres all need rehabilitation personnel in large numbers. Those who are enterprising enough could consider setting up their own rehabilitation centre after some work experience, preferably in collaboration with allied specialists.
Career Progression
In government jobs, there is a time-bound scheme of career progression which ensures that those with similar academic credentials and work experience have equal chances for promotion to the next higher grade. In the area of private practice, or in private health facilities, the quality of work, high degree of interpersonal skills and on-going upgradation can help to ensure a fast track growth. Supervisory and administrative responsibilities are to be expected at senior levels.
Most experienced therapists find excellent opportunities in private practice after having worked in a recognised busy health facility. Corporate medical centres also invite experienced professionals onto their board giving due consideration to academic attainments and quality of work experience.
